Adult Day Services is a licensed program at Hutchins Street Square for older and disabled adults who cannot fully care for themselves. Cancer-diagnosed patients, caregivers, and their families
American Cancer Society provides support to cancer patients and their families at no direct cost to patients or the people who care for them. This includes transportation to medical appointments, transportation reimbursement, lodging, support groups and wigs.

California Poison Control System (CPCS) is the primary source of poison information and treatment advice for all California residents. Specially trained pharmacists, nurses, toxicologists, and health care providers quickly answer calls to 1-800-222-1222 and are available 24 hours a day, 7 days a week, 365 days a year.
All calls are free, confidential, and language interpreters are available in over 200 languages. Callers receive personalized treatment advice and/or a referral for medical care. Free health education materials can be ordered at www.calpoison.org.
Californians in need of dental care, including anyone having difficulty accessing dental care, uninsured, underinsured, unemployed, and adults and children of any age.
CDA Cares is to improve the oral health of all Californians by supporting the dental profession in its efforts to meet community needs. Oral health is critical to the total health and well-being of an individual. For young children, it means they won’t live with pain or miss school. For adults, it can enhance employment outcomes, improve their overall health and quality of life and, for many, restore dignity.
CDA Cares facilitates events and grant opportunities to community-based organizations and individuals who reduce barriers to care in underserved populations. The dental profession and its philanthropic values is represented as it generously gives back to help meet the needs of Californians. Hospice of San Joaquin believes that everyone deserves a comfortable and peaceful closure. The professional team offers a comprehensive care plan which includes professional medical, social, spiritual and emotional care (support) for the patient, family and caregivers. Each patients individualized plan of care is designed to fit specific needs and choices. Visits are as patient and family require or request by the appropriate hospice team member. A 24-hour on-call hospice nurse are available.
Respite care, support and practical help are offered. Counseling, spiritual support and grief support are also included. Some workshops have a one-time materials fee.

Mary Magdalene Community Services offers services to adults, youths and children. Adult services include crisis intervention and referrals, transitional housing and case management, job skills, resume writing, and outreach. Services for youth and children include counseling, case management, and teen mentoring through the "My Brothers Keeper" program. Medi-Cal Access Program (MCAP) offers low-cost health coverage for mid-income pregnant women. It has been designed for middle income families without health insurance and whose income is too high for no-cost Medi-Cal. MCAP is also available to those who have health insurance if their deductible or co-payment for maternity services is more than $500. If you qualify for MCAP, your baby is automatically eligible for enrollment in the MCAP Infant Program unless your infant is enrolled in employer-sponsored insurance or no-cost Medi-Cal. Children grade K-6 whose family lives in a shelter within San Joaquin County and are without permanent housing or are in transition.
one.Charter Elementary is a kindergarten through sixth grade charter school. The school also serves children in transition or homeless children. The schools goal is to satisfy the physical, emotional and educational needs of children in San Joaquin County, using a strong academic program that includes standards-based education with a strong emphasis on literacy and math skills. Children are provided with breakfast, lunch, school supplies, clothing, and other necessities.
Counselors, interns and a school clinician are available to assist children through traumas associated with neglect, abandonment, physical, emotional, and sexual abuse as well as homelessness, hunger, physical and educational issues. The school also serves as a resource center for all homeless youth in San Joaquin County.

Bristol Hospice-Stockton works together with patients and their families to enhance quality of life. Hospice services are available for those typically affected by any type of life-limiting condition. By utilizing the most modern pain and symptom management techniques combined with a compassionate support system, hospice services help to reduce the overwhelming stress and anxiety that can come with caring for a loved one with a life-limiting illness at home or in a residency facility.
Hospice Care Team: When the focus of medical care changes from cure to comfort, the Hospice Care team provides physical, emotional, practical, and spiritual support for the entire family. Hospice provides home visits by nursing staff, medical social services, therapy services, physician services, hospice aide, and volunteer services.
Bereavement/Spiritual Counseling: Hospice also includes individual or group counseling. The professional team can provide individual counseling as well as offer regularly scheduled community groups. Hospice has developed the "Kids Grief Connection" for children experiencing the loss of a friend or a loved one, including a family member. All services are provided to the community free of charge.

Pregnancy Connections Clinic is an open-access prenatal clinic serving pregnant people who are unhoused, using substances, and/or have been diagnosed with syphilis. Pregnancy Help Center of Manteca offers free counseling, pregnancy testing, parenting classes, post-abortion counseling, referrals to other agencies, maternity and infant clothing, baby equipment, diapers, and formula to pregnant women who are in need. Services are available until infant reaches 12 months of age. Parenting classes are available, "Earn-While-You-Learn." In addition, mentoring and counseling services for young men are offered.

Access Managed Care provides telephone linkage to services for individuals in need of mental health and/or substance use treatment. A clinical staff will speak to the caller or caregiver requesting services in order to determine need and eligibility. Upon completion of screening process, individuals can be referred to specialty mental health services, Non specialty mental health services, a substance use treatment program or their identified mental health provider through their health plan.
Programs are funded by the state and federal governments and eligibility is based upon their guidelines.
San Joaquin County Human Services Agency is the communitys provider of those services essential to the well-being of county residents. While all of the programs are locally administered, most are federally and/or state-funded. The mission is to lead in the creation and delivery of services that improve the quality of life for the community. Human Services Agency provides a variety of programs and services locally. Black and African-American pregnant and parenting women, age 16 years and older, and up to six months postpartum.
Black Infant Health is designed to encourage and support a healthy pregnancy, healthy newborn, and women’s health. Twenty group intervention sessions are provided (ten sessions prenatally, ten sessions postpartum), as well as complementary case management.

California Children’s Services (CCS) is a statewide program that arranges, directs, and pays for medical care, equipment and rehabilitation when these services are authorized by the program. CCS pays for treatment, such as doctor services, hospital and surgical care, physical and/or occupational therapy, laboratory tests, X-rays, orthopedic appliances, and medical equipment as needed for the childs CCS-eligible condition.
Child Health and Disability Prevention Program contracts with medical providers who provide complete health assessments for the early detection and prevention of the diseases and disabilities in children and youth. The program staff members work collaboratively with health care providers to ensure that eligible children receive quality health care. Families are assisted with finding a primary care physician and dentist.

Childhood Lead Poisoning Prevention Program (CLPPP) provides case management services to lead poisoned children in San Joaquin County. This program also works with local organizations to educate the community about the dangers of lead poisoning and assists the medical community in identifying and effectively treating lead poisoned children.

San Joaquin Dental Society is a resource for oral health news and updates and can help with referrals to find a local dental provider.
San Joaquin General Hospital general acute care facility providing a full range of inpatient services including General Medical/Surgical Care, High-Risk Obstetrics and Neonatal Intensive Care, Pediatrics and Intensive Care. In addition to the 196+-bed hospital, the medical campus includes multiple facilities dedicated to comprehensive outpatient services including Primary Care and Specialty Clinics. The Hospitals Ambulatory Care sites, including offices located at French Camp and Stockton, provide over 1,340,000 outpatient clinic visits a year.
In addition to providing direct medical services, the hospital is active in providing education for health professionals through post-graduate residency programs in General Surgery, Internal Medicine and Family Practice and has trained over 3,000 physicians since the residency programs were established in 1932. The hospital also participates in clinical affiliation agreements for training programs in a variety of health professions including Registered and Licensed Vocational nurses, Pharmacists, Radiology Technologies, Social Workers and Respiratory Therapists.
The Volunteer Office of San Joaquin General Hospital interviews and places volunteers in the hospital. Teens who are willing to make a six-month commitment are welcome to volunteer working the book cart, front information desk or gift shop. In addition, the Volunteer Office has introduced a new volunteer program, called "Las Teresitas" modeled after the teachings of Mother Teresa. The name Teresita is a befitting name for a program that, at its heart, contains compassion, love, respect and a sincere dedication for humanity. San Joaquin Health Centers (SJ Health) Ambulatory Care Services provides primary care services for both children and adults. Three residency based clinics offers sub-specialty care management in geriatrics, OB/GYN and diabetes. Group classes and minor procedures are also provided. On-site retinal examinations are available daily.

Stockton PACE is an integrated health plan and care team that provides medical and social services to keep the frailest elderly living in the community. An interdisciplinary team of doctors, nurses, social workers, rehab specialists, and aides are provided to meet the participants needs in the facility and in the community.

Stockton Pregnancy Control Medical Clinic provides elective abortions up to 14 weeks. The fees vary depending on length of pregnancy and if anesthesia is chosen. In addition, the RU-486 pill, the Plan B pill, sonograms and pregnancy tests are available. Now accepting new patients for Family Planning Services which include birth control and PAP smears. Testing for STIs are also available for females and males. Walk-ins are welcome.

Stockton Scottish Rite of Freemasonry provides community services in the form of college scholarships and interest-free college loans. Scottish Rite also sponsors a clinic for childhood language disorders where no fees are charged, although voluntary donations from parents and the public are accepted.
